
Form A is used to report quarterly statistics of imports and exports of narcotic drugs.
Form B is used to furnish annual estimates of requirements for narcotic drugs, manufacture of synthetic drugs, and cultivation of the opium poppy, the cannabis plant and the coca bush.
Form C is used to report annual statistics on the manufacture, consumption, utilization and stocks of substances controlled under the Convention on Narcotic Drugs of 1961.
Should a country require a modification to their estimate they may submit, at any time, a change using the form Supplement to Form B.
Form P is used to report annual statistics on the manufacture, trade and consumption of substances controlled under the Convention on Psychotropic Substances of 1971.
Form A/P is used to report quarterly statistics on the trade of substances in Schedule II the Convention on Psychotropic Substances of 1971.
Form B/P is used to provide INCB information regarding a countries need of substances in Schedule II, III and IV the Convention on Psychotropic Substances of 1971.

Form D is used to report annual information on Substances Frequently Used in the Illicit Manufacture of Narcotic Drugs and Psychotropic Substances.
Forms A, B and C are mandatory reporting instruments under the 1961 Single Convention on Narcotic Drugs.
Form D is a mandatory reporting instrument submitted annually to INCB, under article 12 of the 1988 UN Convention against Illicit Traffic in Narcotic Drugs and Psychotropic Substances.
Form P is used to report annual statistics on the manufacture, trade and consumption of substances controlled under the Convention on Psychotropic Substances of 1971.
